We support GP practices in Warrington Central East, improving patient care through collaboration, planning, and shared services.

Warrington Central East Primary Care Network (PCN) is a collaborative group of five GP practices working together to improve healthcare services for communities across central Warrington.

We serve a diverse population and aim to deliver more integrated, proactive, and personalised care by sharing resources, expertise, and innovation across practices.

Member practices include: Cockhedge Medical Centre, Fairfield Surgery, Greenbank Surgery, Holes Lane Surgery and Manchester Road Surgery.

Coordinate care across practices to improve patient outcomes
Introduce multidisciplinary roles such as clinical pharmacists, care coordinators, and social prescribing link workers
Support long-term condition management, vaccination programmes, and health promotion initiatives
Strengthen data-driven planning and patient engagement to enhance service quality
